Identity & Authorization

  • Driver’s license (both sides)
  • Social Security card (if available)
  • Signed engagement/POA forms

IRS Notices

Any IRS/State letters, collections notices, CP/CP2000/CP14 letters, wage garnishment or levy notices

Income & Financials

  • Recent pay stubs
  • Prior-year tax returns
  • Bank statements (last 3 months)
  • 1099s/W-2s
  • Profit & loss (if self-employed)

How It Works

STEP 1
1) Identify your case

Enter your name and (if you have it) your Case/Client ID so we can match files instantly.

STEP 2
2) Choose Files

Upload your files below, click “Choose Files” to select from your computer or phone.

  • Accepts PDF, JPG/PNG images, DOCX
  • Upload up to 6 files at once
  • Each file can be up to 6 MB in size
  • You’ll see each file listed as it uploads

Tip: If your file is larger than 6 MB, split it into parts (e.g., multiple PDFs) or compress it before uploading.

STEP 3
3) Submit

We’ll confirm receipt on screen and by email. Your case manager is notified automatically.
